The COSEHC Quality Impact Practice Transformation Network (PTN) Clinical Action Program was designed to assist practices in executing a formal quality improvement process to optimize patient health outcomes. Through this engagement, QualityImpact worked with providers and their healthcare teams to analyze clinical data and identify improvement opportunities. An action plan was designed by the practice aimed at closing gaps in care and improving performance.
This program ran for 6 months from the practice's start date. To receive CME credits, physicians must register and agree to execute the practice-developed action plan.
The Clinical Action Program requires the following:
Assessment (5 CME Credits)
Intervention (5 CME Credits)
Evaluation (10 CME Credits, if all 3 phases completed)
